Payment of co-pays, deductibles and co-insurance are expected at time of service. We accept cash, checks, MasterCard, Visa and Discovery.

Understanding your health insurance plan’s benefits and restrictions has become a very difficult task in these days of managed care programs. We shall endeavor to assist you as much as possible however it does remain your responsibility to be aware of the details of your particular policy. Some of the limitations for you to be aware of may include:
Utilizing a specific lab or pharmacy
We have an “in house” lab that is contracted by Quest Laboratories. If your insurance company does not cover labs from this company, you will need to request to be sent to another company’s laboratory for your ordered lab tests. Please check with your insurance company before you have your labs drawn, otherwise you could incur additional fees.
Prior authorizations or referral to be evaluated by a Specialist
Certain insurance companies require that you obtain prior approval before being sent to a specialist provider or have specialized tests that are not performed by our physicians or staff within our clinic. Usually we can obtain an authorization within one or two working days, but sometimes it can take slightly longer.
Some insurance companies “assign” a primary care provider. You should know the provider to whom you have been assigned. By being assessed and treated by a provider to whom you are not assigned, you are liable for any claims and/or bills that are not reimbursed by the insurance company.
Not following your insurance company’s specifications can result in a patient/family being responsible for payments that their insurance plan would normally cover. Please review your Plan Summary carefully, and contact your company’s Member Services department with any questions. Remember, your insurance coverage is a contract between you and your insurance company. Northside Family Practice is not responsible for services denied by your insurance company.
